HOW TO FILE A COMPLAINT

Members of the public and Defense Information Systems Agency (DISA) employees; including military members, civilian employees, and DISA contractor employees; may file an unclassified complaint with the DISA Office of the Inspector General (OIG) Hotline in one of five ways listed below.

Online Form
The online form provided will allow you to submit your complaint. This form is for unclassified complaints only. Please follow the instructions provided if you are submitting your complaint through the online form.

Email 
You may email your complaint to disa.meade.ig.mbx.ig-hotline@mail.mil. When emailing your complaint, please include your name and contact information, allegation details, subject's name, and your consent to disclosure.

Letter
You may mail your complaint to:

When submitting your complaint, please include your name and contact information, allegation details, subject's name, and your consent to disclosure.

Fax
You may fax your complaint to (301) 225-0514. When faxing your complaint, please include your name and contact information, allegation details, subject's name, and your consent to disclosure.


Telephone
At any time you may contact the DISA OIG Hotline at (301) 225-6250 (DSN 375-6250). However, we encourage you to submit the allegation(s) in writing by e-mail, fax, letter, or using the online complaint form.  Our experience has shown that written complaints are more organized and provide more details, and based on experience, we have found this to be the best way to serve you.
